Revision Forms

If you would like to request to have your financial aid revised, you may use one of these forms below. Because they all require signature and may require additional documentation, you must print, complete and mail these forms to our office. Each request will be considered but is not automatically approved. Be sure to include all required documentation.

Many of our financial aid forms are aid-year dependent. When selecting a form be sure to use the aid year appropriate for your situation.

Revision Forms

Cancel & Reinstate Aid	Use this form to cancel or reinstate financial aid, including student loans.
Class Level & Residency	Your aid eligiblity could be affected if your college enrollment status or WA residency status changes.
Enrollment Revision	Report expected enrollment status changes (eg. 1/2 or 3/4 time, full time) to avoid aid delays.
Computer Purchase	Increase your direct loan in order to purchase a computer.
Contribution Adjustment Request Student \| Parent	Report unusual circumstances affecting parent financial resources.

Verification Forms

Dependent Worksheet Verification	Required when requested for dependent students and parents in order for federal aid to be awarded.
Independent Worksheet Verification	Required when requested for independent students in order for federal aid to be awarded.
Asset Information Student \| Parent	When requested, report asset information.
Income Verification Student \| Parent	When requested, report income and expense information.
Household Size Verification (Independent Student)	Verify the number of people living in your household when requested.
Household Size Verification (Dependent Student)	Verify the number of people living in your household when requested.
Housing Status Update	Indicate your housing status if you did not submit it on FAFSA.
Income Exclusions Student \| Parent	When requested, report sources of income excluded from financial aid consideration.

Loan Forms

Grad PLUS Loan Application	For Grad Students who have exhausted direct loan eligibility.
Loan Revision Authorization	Allows for an adjustment of a loan without financial aid having to seek permission each time.
PLUS Loan Addendum	If a parent wants to borrow a parent loan for the student.
PLUS Loan Denial	If a parent is denied the PLUS loan, the student may be eligible for an additional unsubsidized loan.
PLUS-Only Loan Application	Use this form if you do not intend to receive student student financial aid- FAFSA not required.

Petition Forms

Maximum Timeframe Petition	Students who have reached the mazimum allowable credits for aid eligibility may petition for additional time.
Satisfactory Academic Progress Petition	Students who do not complete enough credits to be considered eligible for financial aid may petition to regain aid eligibility.

Assorted Forms

Dependent Care Expense Worksheet	Students who pay for out-of-pocket child or dependent care expenses may request additional aid.
Discontinue Financial Aid	Cancel all financial aid.
Postbaccalaureate Verification	Get a Federal Direct Loan as a postbac student.
Veteran's Benefits	Required verification of your veteran's educational benefits.
